Detailed project description: detailed explanation of the proposed program
Project goals and objectives with timeline
Proposed use of award funds
Measures of project’s effectiveness
Proposed measures to sustain your program’s efforts
Proof of IRS federal tax exempt status, 501(c)3 and W9
Roster of board of directors and executive leadership team
Operating budget, profit and loss statement
